Gaston County Restaurant Inspections Feb 26-28 2008

The Gaston County Health Department has inspected the following food establishments: Ratings are as follows: A, 90-100; B, 80-89; C, 70-79; below 70, closed until grade above 70. The businesses were inspected between Feb. 26-28, 2008. Comments are from inspectors.

CRACKER BARREL, 1821 REMOUNT ROAD, GASTONIA, 83.5

Store and handle food in or on clean area and in a manner to prevent contamination. No food storage on the floor! Several cases of food products stored on floor in walk-in cooler. Store food products on shelving or transfer cart. Do not store or place food products in standing/stagnated water in base of refrigeration units. Store food products in walk-in cooler or reach-in refrigeration on higher shelving. Do not store food in one container directly on top of food in another container. Container of onions stored directly on top of nuts in another container. Cover food products before storing container of food products atop another food container. Minimize the amount of food products placed in pans on main cook line. Container of food products overflowing with food products and the food products are rubbing/touching interior of reach-in freezer. Keep food products covered at all times! One food prep/operation at prep table/prep sink at any given time. Employee had raw bacon on prep table while removing cooked chicken from oven.

Store opened packages of dry food products in sealable container labeled and kept clean (grits, flour, etc.). Keep lid on dry storage container/bins. Relabel dry storage bins/containers.

Thoroughly clean all food contact surfaces. Unclean utensil/equipment cannot be properly sanitized. Upon arrival of inspection, no sanitizer was registering in sanitizer bucket throughout kitchen area. After priming unit at three-compartment sink and getting air bubbles out of line, chemical dispenser provided properly mixed sanitizer - use sanitizer test strips. Repair dishmachine immediately. Dishmachine is not sanitizing on sanitize cycle. Until dishmachine is repaired, wash, rinse and sanitize utensil/equipment in three-compartment sink. Thoroughly clean interior of ice machine and corners of prep sinks. Mold/slime present in corners of prep sink, on underside of head to ice machine and ice guard inside ice machine. Thoroughly clean all tape from containers. Rewash all utensil/equipment in kitchen area because establishment does not know when dishmachine stopped sanitizing!

Thoroughly air-dry utensil/equipment before stacking or storing. At least, attempt to air-dry. Clean utensil/equipment shall be stored in or on clean area in a manner to prevent contamination. Thoroughly clean all utensil shelving and storage areas throughout entire establishment! Do not spray floor with spray hose around utensils. Employee spraying floor in back kitchen area and water splashing and splattering on utensils on lower utensil shelving. Clean; clean end of drainboard and keep clean at all times! Do not store or place lids to cover top section of refrigeration units on the floor. Store on shelving or top of equipment. Do not store or place ice bucket on the floor - replace ice bucket - do not use old chemical bucket for ice bucket.

Store and handle single-service articles in or on clean area and in a manner to prevent contamination. Clean case of Saran Wrap holder. Stagnated water, slime and food debris present in base of Saran Wrap holder. Do not store to-go box/carry-out boxes on the floor in back storage room.

Repair/replace salad refrigeration unit. Clean shelving throughout entire kitchen, walk-in cooler and storage room. Clean underside of shelving over prep tables. Clean underside of mixer. Clean base/underside of tilt-top kettles. Clean drain trough to kettles. Clean timers. Clean doors to walk-in cooler/freezer. Repair loose lines inside refrigeration Units. Detail clean equipment on main cook line. Clean around door/drawer thresholds on refrigeration units. Repair busted doors on freezer units. Clean door to ice machine. Clean lid to cover ice bins. Repair prewash end of dishwashing line to NSF standards. Prewash end of dishwashing line is cracked/broken and spill sink and metal conveyor to push racks through. Detail clean biscuit holder. Detail clean all equipment in back prep area! Thoroughly clean ovens. Clean soda in-a-box stand at lines.

Keep dumpster door closed at all times!

Patch holes in wall. Replace broken, damaged, missing floor tiles! Re-grout floor throughout where needed (kitchen area, dishwashing area and walk-in cooler). Clean floors, walls and ceiling throughout entire establishment! Clean wall under drainboards in dishwashing area. Repair wall under drainboards and anywhere else needed.

Provide light shield over light fixture missing light shield. Replace cracked/damaged light shields.

Clean ventilation under hood system.

Label chemical bottles in dishwashing area. Store toxic substance in designated area. Do not store or place containers of chemicals for dishmachine on shelving over clean end of drainboard. Store chemicals in designated area.

Clean underside and inside of men’s wall urinal. Proper ice-bathe contents of ice in bottom container coming in contact with base and sides of food container up to the height of the food products.

Do not remove or obstruct grade card. Grade card must stay posted at all times! Once violations have been corrected, may call for re-grade. Removal or obstruction of grade card will result in legal action in Gaston County Court of Law.

PETRO EXPRESS, 2106 N. CHESTER ST., GASTONIA, 85.5

Maintain potentially hazardous hot food products at 135F or above and potentially hazardous cold food products at 45F or below. Tornadoes - 125F on hot dog roller. Coleslaw 49F.

Provide thermometer 0F-220F at all times. Need thermometer to check temperature.

Thoroughly clean ice dispenser. Slime and green stuff hanging from nozzle.

Clean shelving throughout. Clean area under cabinet/counter spaces. Clean table where drink dispenser is stored. Clean shelving in walk-in cooler where food products are stored. Wipe/clean soda in-a-box stand and line.

Provide hot water in bathroom area. No hot water at hand sinks in bathroom, but has hot water at hand sink and three-compartment sink in back kitchen area near register.

Re-attach paper towel dispenser to walls. Provide correct soap bag for soap dispenser. Thoroughly clean all bathroom fixtures (hand sinks, toilets and hand-drying devices).

Clean floors, walls and ceiling throughout. Thoroughly clean walls in bathrooms.

Provide light shield with end caps over light fixture over three-compartment sink.

Clean ventilation in bathrooms.

Organize all storage spaces/areas. No storage on the floor. Clean grounds around dumpster corral and outside storage shed.

PIZZA HUT CARRYOUT, 2521 E. OZARK AVE., GASTONIA, 86.5

Store and handle food products in or on clean area and in a manner to prevent contamination. Store raw eggs below and away from other food products in walk-in cooler. Do not store or place container of food product in standing water in base of refrigeration unit. Have pizza p rep line repaired immediately. Water should not be collecting in base of refrigeration unit. Eggs placed on lower shelving in walk-in cooler. Container of food product removed from pizza prep line refrigeration unit.

Thoroughly clean food contact surfaces. Unclean utensil/equipment cannot be properly sanitized. Thoroughly clean individual vats of three-compartment sink. Repair dishmachine immediately! Dishmachine is not sanitizing on sanitize cycle! Until dishmachine is repaired, wash, rinse, sanitize utensil/equipment in three-compartment sink. Ran several racks through dishmachine; still not sanitizing. Monitor sanitizer strength in spray bottle. Found one sanitizer bottle (chlorine) close to 200ppm.

Thoroughly air-dry utensil/equipment before stacking or storing. Clean utensil/equipment shall be stored in or on clean area and in a manner to prevent contamination. Thoroughly clean utensil shelving throughout.

Repair three-compartment sink to NSF standards or replace. Three-compartment sink has a crack on prewash end of sink at drainboard and upper vat of sink. Also has a crack on clean end of drainboard at sanitizer vat. Either properly repair or replace three-compartment sink, or an intent to suspend will be issued on next visit, giving establishment 30 days to get sink repaired or replaced. Clean edge of door and door handle on upright freezer units. Clean door to walk-in cooler. Thoroughly clean shelving in walk-in cooler. Clean gaskets on equipment. Clean around door threshold on pizza prep line unit. Detail clean speed racks/dollies. Clean monitors. Replace missing fan cover in pizza prep line refrigeration unit.

Repair floors, walls and ceiling throughout establishment. Repair walls in bathroom and near register area. Repaint throughout. Clean back door. Clean storage room door. Repair damaged floor tiles under dishmachine. Clean floor around/near can wash and grease trap. Replace broken/damaged floor tiles.

Repair light fixture near walk-in cooler. Replace blown light bulbs under hood system or repair light fixtures.

Clean ventilation under hood system and fan cover in walk-in cooler.

No storage on racks or crates. Purchase NSF dunage rack for storage of extra supplies/stock. Organize chemical closet/room.

Do not remove or obstruct grade card. Grade card must stay posted at all times. Once violations have been corrected, may call for re-inspection. Removal or obstruction of grade card will result in legal action in Gaston County Court of Law.

ASAHI EXPRESS, 3033 S. UNION ROAD, GASTONIA, 91.5

Rice - 60-110 degrees. Critical item. White rice was thrown away. Fried rice was re-heated to 165 degrees. Quick-chill rice in freezer or keep above 135 degrees.

Store spatulas in 135-degree water at grill. Air-dry all containers and utensils before storing.

Need to remove cardboard from under grill. Clean under woks and grill.

Need to repair self-closure on women’s restroom door.

Need to properly label all chemical bottles.

CDA STORE, 6909 S. OAKLAND ST., GASTONIA, 94

Remove deer meat from premises. Packages of deer meat stored in upright freezer unit in main kitchen area with other food products used in kitchen.

Clean utensil/equipment shall be stored in or on clean area and in a manner to prevent contamination. Wipe/clean utensil shelving and storage area. Wipe/clean base of utensil bin. Clean; clean end of drainboard on two-compartment sink and keep clean at all times. Do not hand-dry utensils with towels; air-dry!

Repair or replace door to ice machine. Wipe/clean shelving under front counter. Clean control knobs to equipment. Replace missing control knob to equipment. Clean pivot/hinge area on deep fryer. Replace split/broken gasket on refrigeration unit. Wipe base of refrigeration units. Clean spray nozzle and faucet fixture on two-compartment sink.

Clean and repair floors, walls and ceiling throughout.

TIMES TURN AROUND, 3050 UNION ROAD, GASTONIA, 94.5

Need to clean the front of the cappuccino machines in front of the spouts.

Do not store anything in employee handwash. It is for handwashing only and must be easily accessible for employees’ use. There were drink bottles in and on the employee handwash in the kitchen.

Need handwashing signs at restroom and employee handwash in kitchen.

Keep dumpster doors closed when not in process of emptying refuse into dumpster.

The cove base in the hallway in front of the restroom is coming loose from the wall. It needs re-attaching.

MCDONALD’S, 1700 S. YORK ST., GASTONIA, 94.5

Employee drinks shall have lids and straws; stored below and away from food, food prep area, utensil, utensil storage area, single-service articles and clean linens. Employee drink stored in reach-in with no lid or straw, stored above food products. Store employees’ personal belongings in designated area. Employee’s purse, coat and newspaper stored in biscuit-making area near single-service articles.

Thoroughly clean interior of ice machine. Remove ice guard/shield from interior of ice machine and clean behind ice guard (clean underside of head of ice machine).

Store and handle single-service articles in or on clean area and in a manner to prevent contamination. Do not store or place single-service articles on the floor. Racks and crates are a temporary means of storage. Purchase NSF dunage racks to store case of single-service articles and Happy Meal toys on. Hang bags of gallon jugs on wall in outside storage room. Clean condiment bin at drive-through.

Wipe/clean gaskets, base and shelving in reach-in refrigeration unit. Repair leaking faucet fixture on three-compartment sink. Wipe/clean shelving throughout kitchen, storage room and walk-in cooler. Replace broken, split gasket on walk-in cooler door. Replace damaged door to ice machine (door separating - exposing fiberglass). Replace cracked, damaged trays. Wipe/dust top of monitor (register area). Replace missing faceplate on top of drink dispenser.

Repair missing/damaged floor tile in dishwashing area. Patch/seal holes in wall and repaint (storage area/laundry area/drink dispenser machine, in back). Clean floors, walls and ceiling throughout where needed, if needed.

BOJANGLES, 400 E. GARRISON BLVD., GASTONIA, 95

Maintain potentially hazardous cold food products at 45F or below. Food products in reach-in near biscuit-making area 47F-49F. Food products in walk-in cooler 49F. Reach-in electrical cord was plugged into an electrical outlet that was not working. Electrical cord plugged into a different outlet and temperature of food products was 45F and below. Mark outlets that work or repair outlets that don’t work. Repairman was called at time of inspection to come work on walk-in cooler.

Employees shall wear effective hair restraints.

Clean utensil/equipment shall be stored in or on clean area and in a manner to prevent contamination. Thoroughly clean utensil shelving over two-compartment sink. Do not store lids to cover deep fryers on the floor. Store on lower shelving under prep tables.

Clean doors to walk-in cooler/freezer. Thoroughly clean shelving in walk-in cooler. Clean soda in-a-box stand and lines. Clean door to ice machine. Replace broken/damaged chemical dispenser on prewash end of dishwashing line. Clean spray nozzle on prewash end of dishwashing line. Re-attach spray nozzle holder to wall. General detail cleaning of equipment on main cook line. Clean control knobs to equipment. Clean underside of hot holding unit over biscuit-holding area.

Clean floors, walls and ceiling throughout where needed.

Clean vents under hood system.

No storage on racks or crates! Purchase NSF dunage rack for additional storage in walk-in cooler and area in front of ice machine to store ice buckets on. Do not store/place ice buckets inverted down on milk crates.

BI LO DELI, 2204 UNION ROAD, GASTONIA, 95

Need to dust the walk-in cooler fan guard.

The left handwash in bakery section drains slowly.

The broken bulb shield in front of the fryer need replacing.

Some ceiling tiles are stained and need replacing; i.e., above the fryer.

SAKE EXPRESS, 116 N. NEW HOPE ROAD, GASTONIA, 95

Need to label sanitizer containers beneath front counter. Critical item - corrected on site during time of inspection.

There was a bag of refuse lying in the dumpster enclosure behind the dumpster. Put all refuse into the dumpster.

Some kitchen ceiling tiles are stained/water damaged and need to be replaced.

Need to clean restroom exhaust vents. Need to replace the cracked and stained light shield above the True two-door upright refrigerator.

All storage should be 12 inches above floor on approved shelving. This is in the storage room beside the employee entrance.

GONDOLA ITALIAN RESTAURANT, 610 GASTONIA HWY., BESSEMER CITY, 95.5

Need to let all dishes air-dry. Observed employee drying dishes with a towel.

Invert single-service spoons.

Need to clean inside refrigeration and microwaves. Replace torn gaskets on reach-in cooler.

Need to clean floor in walk-in freezer.

Area needed to store personal items.

CHINA GOURMET, 2211 N. NEW HOPE ROAD, GASTONIA, 96

When foods are to be thawed (from a frozen state to a non-frozen state), move them from freezer to cooler overnight, thaw in a microwave (half power), place under cool running water in a prep sink (when there are no water restrictions).

Air-dry all foodware by cross-stacking or laying foodware on wire shelves before they are stacked for storage. Scoops, spoons or measuring cups with handles are needed in dry food bins. Always keep handles up out of the dry food bins.

THE FAMILY PIZZAGALLI, 1219 GASTONIA HWY., BESSEMER CITY, 96.5

Vegetable prep sink is for food prep only. Pre-wash dishes in three-compartment sink. Critical item.

Hair restraints required.

MILANO RESTAURANT, 1011 E. CHERRY ST., CHERRYVILLE, 96.5

Slicers, cubers and choppers need to be broken down and run through dishwasher instead of rinsing with hose and wiping with a towel. Critical item.

Need to clean inside freezer.

SUB STATION II, 802 E. FRANKLIN BLVD., GASTONIA, 97

Non-NSF equipment was present. All equipment should be NSF-approved. Three chest freezers are not NSF-approved.

Need to air-dry plastic prep containers before stacking/storing.

Walk-in cooler gasket needs replacement. Need to clean walk-in cooler fan guard. Prep cooler gaskets need replacing. Also, the bottom of the prep cooler needs cleaning. Need to clean the left door gasket on the cooler near the cash register.

Wallpaper in men’s restroom is peeling. Some ceiling tiles are stained in men’s restroom. The storage room entrance wall is scratched/peeling.

LOWELL MINI MART, 719 MCADENVILLE ROAD, LOWELL, 97

More detailed cleaning and sanitizing is needed, both inside the ice machine and self-serve ice chute on drink machine on a weekly basis minimum.

Two-door warmer, sandwich make table cooler and walk-in coolers all need door gaskets that make a good seal on door and that are not cracked, where applicable. Dust all self-serve line - tops, sides, underneath are all dusty. Dust wire shelves dry food bins (sugar, beans, breading and cornmeal). Clean crumbs out of prep table utensil drawer.

Dust all fan guards and fan guards inside walk-ins. Paint on hood is chipping/peeling and needs paint.

Drill holes through sandwich make table cooler cutting board and flip over - top side is beginning to show wear.

Keep dumpster doors closed at all times.

PISGAH A.R.P. CHURCH, 3600 S. LINWOOD ROAD, GASTONIA, 97

Sneeze guard needed for salad bar. No open display of food. Critical item. Call health inspector when corrected.

Hair restraints required.

QUIZNO’S SUBS, 2524 #A E. FRANKLIN BLVD., GASTONIA, 97.5

Keep Quaternary Ammonia sanitizer at minimum strength of 200ppm at all times in buckets. More detailed cleaning and sanitizing is needed on slicer after preparing/slicing meats. Don’t allow soiled knives to be left out for multiple uses; clean immediately.

Stack single-use knives, forks and spoons such that only handles are touched when picked up and not eating ends.

SUBWAY, 3826 S. NEW HOPE ROAD, GASTONIA, 97.5

Sanitizer dispenser was not working properly. It was dispensing Quaternary Ammonia sanitizer at about 100ppm. The minimum required strength is 200ppm. Employees began using chlorine sanitizer at minimum strength of 50ppm until dispenser is repaired. Health department will return in 10 days for compliance. More detailed cleaning is needed on drink dispenser nozzles and outsides of cambros where date dots are. These must be cleaned because when stacked within one another, they touch food contact surfaces of next cambro.

Do not overstock tray meats over chill line because they will not hold 45F and less temperatures in display cooler.

Dust floor fan guard in back.

LOWELL FAMILY RESTAURANT, 125 N. MAIN ST., LOWELL, 97.5

Do not remove or obstruct grade card. Once violations have been corrected, may call for re-grade.

HILLBILLY’S BARBECUE & STEAKS, 720 MCADENVILLE ROAD, LOWELL, 97.5

More detailed cleaning and sanitizing is needed on slicer in pit area. Corrected on site.

Use scoops or spoons with handles in sugar/cinnamon mix and in hush puppy mix - don’t use cups or bowls. Corrected on site.

Clean floor under prep tables in kitchen and in wait station.

Use more ice and smaller pans when cooling ribs.

Clean under salad bar in cabinet.

Light shield is needed in employee restroom.

Make sure that all sanitizer buckets are at minimum strength - use test strips; for chlorine, the minimum strength is 50ppm. It is better not to mix dish liquid with sanitizers. Do not store sanitizer bucket in meat prep sink. Wash all foodware in three-vat sink and not hand sinks. Keep hood screens up at all times. Keep food scraps and mop water up out back. Self-close back door.

FOOD LION DELI, 202-A MARKET ST., CRAMERTON, 97.5

Keep potentially hazardous foods at 135F and above and at 45F and below, or discard after two hours. Always adhere to holding times of chicken. Make sure if you hold for more than two hours, that chicken must be 135F and above. If not, discard. Always mark discard times on chicken labels.

Clean walk-in cooler door gasket - mildew/staining.

FOURSHEE’S, 6231 DALLAS-CHERRYVILLE HWY., BESSEMER CITY, NC, 98

Need to label sugar container.

Hair restraints required.

FOOD LION, 3133 DALLAS-HIGH SHOALS ROAD, DALLAS, 98

Food prep sink is for food prep only. Observed employee using prep sink as a dump basin for out-of-date food. Critical item.

Cup dispenser needed for single-service cups.

Need to clean floor in walk-in freezer.

WAL-MART DELI, 701 HAWLEY AVE., BELMONT, 98.5

All potentially hazardous foods that are held hot should be held at 135F or hotter. Chicken chunks were 120-125F. Chicken wings were 118-120F. Critical item - corrected on site.

Need to repair spray nozzle for dishwashing sink in bakery area.

HARDEE’S, 716 MCADENVILLE ROAD, LOWELL, 99

Ice cream scoop is pitted and needs replacement; all food-contact surfaces should be smooth and easily cleanable.

Wire shelves in walk-in cooler need cleaning; “cubby-type” shelves where multi-use items are stored need cleaning - crumbs; ice cream cooler gaskets at drive-through need replacement - cracked.

Air-dry all foodware before stacking for storage - in walk-in. Dumpster needs new drain plug - is cracked. Light cover in outside storage needs replacement.

Clean floor under deep fryers.

FOOD LION DELI, 515 HWY. 27 SOUTH, STANLEY, 99

Need to clean/dust some storage shelves.

HARRIS TEETER DELI, 1751 NEAL HAWKINS ROAD, GASTONIA, 99.5

Keep slicers cleaned and sanitized between orders and breakdown totally every four hours.

Clean hood filters.
